To get the information on your player's evaluation skate times, please enter in their date of birth and their main contact phone number. The date of birth MUST be in the following format: YYYYMMDD (for example, a player with the birthday of January 19, 2001 would be entered as 20010119). The phone number must be entered using only numbers (for example, if the player's phone number was (780) 555-1212, the phone number must be entered in as 7805551212).
